![]() | Belinda Hays, PT, DPTOwner/PresidentA native of Seymour, Indiana, Belinda founded Progressive Physical Therapy in 1987 with the goal of providing quality physical therapy services to patients in areas with limited access to physical therapy providers. She is a 1981 graduate of Indiana University and earned her Bachelors of Science Degree in Physical Therapy. In December 2010, Belinda attained her Doctorate of Physical Therapy Degree from the EIM Institute of Health Professions. Belinda is committed to providing compassionate, quality care to patients with various needs of rehabilitation. She believes that it is not only important to assist patients in treating their symptoms, but also giving the patients the resources they need to commit to ongoing physical health and wellness. Belinda has been a practicing physical therapist for over 30 years. She has developed special expertise in the areas of orthopedics, spine rehabilitation, ergonomics analysis, and work injury management and prevention. Belinda has certifications in the ASTYM system and DSI Work Solutions FCAs, Functional Job Descriptions/Analysis, and Post-Offer Work Screens. She is an active member of the American Physical Therapy Association (APTA) and has served on many committees, nationally and statewide. She is a member of the Indiana APTA and has served as a past Membership Chair and a Past Board of Directors Member. She currently is a board member of the Indiana Workers Compensation Institute. Belinda and her two daughters, Taylor and Madison, reside in Seymour, IN. She enjoys spending time with her family. | |

![]() | Jessica Alcock, PT, DPT, MSFacility Manager, SeymourJessica is a 2007 graduate of Boston University with a Doctor of Physical Therapy Degree and is a 2001 graduate of the University of Indianapolis with a Masters of Science Degree in Physical Therapy. She also earned a Bachelor of Science Degree in Psychology in 1999 from the University of Indianapolis. She has been a practicing physical therapist for over nine years, and she joined Progressive Physical Therapy in 2004. Her clinical experience is in outpatient orthopedics treating a wide variety of diagnoses across the spectrum from young athletes to geriatric patients. Jessica is dedicated to providing high quality, individualized services to all of her patients. She believes in a skilled therapeutic approach that incorporates the use of evidence based practice, a strong patient-therapist relationship, and personalized care. Her goal is to help patients return to their normal daily and work activities and to teach them self-management techniques for the future. Jessica has earned certifications in the ASTYM system, as a Clinical Instructor, and in DSI Work Solutions FCAs, Functional Job Descriptions/Analysis, and Pre-Work Screens. She has additional training in urinary incontinence and pelvic pain as well as functional assessment and rehabilitation through courses by Gary Gray, PT. Jessica’s professional interests include sports medicine physical therapy and general orthopedics. She is a member of the American Physical Therapy Association. In her free time, Jessica enjoys yoga, volleyball, and reading. Jessica and her husband, Bill, and son, Ethan reside in Seymour, IN. | |
![]() | Dawn Bolte, DPTFacility Manager, ColumbusDawn is a graduate of Washington University in St. Louis and earned her Doctor of Physical Therapy degree in May of 2006. She also holds a Bachelors degree from the University of Alabama in Health Science that she earned in 2003. Dawn believes in using an evidence-based approach to evaluate, diagnose, and treat patients with musculoskeletal impairments and injuries. Her goal is to provide compassionate care to all of her patients and to help them maximize their functional outcomes. Dawn joined Progressive Physical Therapy at the Columbus clinic in July 2006. She was born and raised in Columbus, and she is excited to have the opportunity to make a difference in her hometown. Dawn is as member of the American Physical Therapy Association and has mentored under a vestibular rehabilitation specialist as part of her clinical experience. She is also a certified provider of ASTYM and DSI Work Solutions Functional Capacity Assessments. Dawn’s professional interests include general orthopedics and vestibular rehabilitation. Dawn and her husband, Craig, reside in Columbus, IN. In her free time, she enjoys running, biking, camping, reading, scrap booking, and playing the piano. | |
![]() | Crystal Henderson, PTFacility Manager, GreensburgCrystal is a 1995 graduate of Maryville University in St. Louis with a Bachelors of Science degree in Physical Therapy. She has been a practicing physical therapist in both Indiana and New York. Crystal has over 16 years of clinical experience in providing physical therapy services in both outpatient orthopedics and workers compensation. She has treated patients of all ages and diagnoses. Her professional interests include general orthopedics, sports related physical therapy, and industrial rehabilitation. Crystal believes in providing all of her patients with personalized, one-on-one care. She is committed to helping patients reach their goals and return to the normal lifestyle by focusing on functional daily and work activities, self-management, and preventative education. Crystal has earned certifications in the ASTYM system and as a Clinical Instructor. She also has specialty training in industrial rehabilitation and in DSI Work Solutions Functional Capacity Assessments. She is a member of the American Physical Therapy Association. In her free time, Crystal enjoys staying active and participating in outdoor activities. She also enjoys spending time with family and friends. Her husband, Ron, and their two sons, Alex and Ryan, reside in Greensburg, IN. |
